You definitely had to kill all 4 heads in the past. However, after the change to make things easier, only a single head needs to be killed now. I don't think the difficulty matters. The only time you have to kill 4 heads now is when you fight the super powered one in the secret shadow mission.
Or at least that's how I remember it.
