Where do the pages drop, and do they only drop from specific mobs in those locations?

Bachram Potential Page 1 Drops in Avalon Bridge. Only one ive found so far.

I had a similar experience. Page 1 dropped for me in Counterattack after I opened the chest at the end of the mission.

Counterattack may drop all pages, actually. I happened to get Page 4 from it. Funny thing is, that was the last page I needed xP

Funny, I actually just caught page 3 from a wolf mob in Knights of the Round Table.

Currently, the Master title is spelled incorrectly in-game. It says "Bacharam" instead of "Bachram". Should it be noted down at the bottom like [sic] or something, or just kept as it should be spelled?

Typically for something like this you should write what's used in game, but it will also probably be changed pretty quickly (assuming there's a bug report for it already), so I'm not too worried with leaving the page as is.
